All About You

All About You is a new disability support organisation in the Huon, operating from Geeveston. They provide support for participation in skills-based learning to develop independence in accessing the community.  This may include assistance to establish volunteer arrangements within the participants home or community to develop skills; recreational activities; or individual skill development such as:

  • life transition planning such as assistance in attending appointments, shopping, paying bills, taking part in social activities and maintaining contact with others
  • group and individual skill development to assist independence in performing daily activities
  • individual social skills development for participation in community and social activities.

Anglicare

Our friendly, highly skilled team can assist people who are eligible for the NDIS to enjoy social activities and connect with the community. Our support will be different, based on what each person wants to do. For example, we can support you to participate in:

  • Social activities and events with family and friends
  • Trying new things or finding people who share your interests. You might want to be part of a community garden, go fishing, learn music, join a club or become a volunteer…the choice is yours
  • Getting out and about. You might like to go to cafes or go to the footy. Some people prefer to go to the movies, or to a concert…the choice is yours

What do you like to do? Whatever it is, our friendly team can provide the supports you choose based on your interests and what you enjoy. Anglicare provides support young children.

Community Based Support

Community Based Support is a not-for-profit organisation that has been supporting people to remain living independently at home for over 30 years!  If you are eligible for the NDIS, we have a friendly and professional specialist Disability Care Team who would love to chat to you about your goals.  We can help you access the community through 1:1 social support and group activities.

Li-Ve Tasmania

Li-Ve Tasmania currently have three main community access programs:

The Cottage: located at New Norfolk
The Zone: located at Oakdale Lodge, Warrane
The Lodge Day Options: located at Oakdale Lodge, Warrane.
The Cottage program caters for community access options for older persons with a disability, whilst the The Lodge Day Options is a social and leisure program located at Oakdale Lodge. The Zone is an innovative program for college leavers and young adults living with disability.

Li-Ve Tasmania models all its supports and programs on individual choice, control and flexibility and Community Access is no different. Li-Ve Tasmania provides support to children.

Mosaic

Our Community Access programs are all designed to give you the chance to learn and develop your life skills. We provide day support, after school programs and programs for people who are leaving school but aren’t quite sure if they’re ready for further education. We also provide Camps in the School Holidays and Vacation Care. Mosaic Support Services is very proud of our arts program, where we teach, among other things, photography, film-making, painting and ceramics. We run the Mosaic Festival of Arts each year which gives our clients the opportunity to exhibit their work in several locations across the city. The festival also includes performances from our bands and dance groups and is a wonderful way to showcase our clients individual talents to the wider community.

OAK-Possability

The Community Connections program of OAK-Possability provides exciting opportunities to participate in a wide range of social activities, physical exercise and community-based activities. Whatever your individual interests, we are dedicated to making it happen.

OAK-Possability assists clients to identify activities in the community that match their interests and then support them to access these activities. This helps reduce social isolation and promotes the development of independent social networks. This style of support can include community-based Day Service programs tailored to the individual.

Community Connections also assists clients to undertake daily tasks in the community, such as shopping, banking and attending medical appointments. It often includes learning to travel independently using public transport.
Where is it available?
We offer Community Connections across Tasmania. Services can be offered in rural areas, in addition to metropolitan and regional centres.

Oak-Possability provides support to young children.

Nexus

Sometimes you might need a little more support to get out and about. Nexus helps you identify the type of activities you want to do and then we help you make it happen. Nexus has the staff to support regular outings like shopping or attending college through to one-off events like rock concerts or interstate travel.

The Parkside Foundation

The Parkside Foundation provides assistance to live and actively connect with your community, to exercise independence and control over your own life.
This may include:
Care co-ordination – improving life choices
Social and community support
Assistance with daily living skills
Individual planning
Tenancy assistance
Budgeting, literacy, numeracy
